A partner of the Optum Care network, WellMed Medical Group, is seeking a nurse practitioner or physician assistant to join our Home and Supportive Care team in the Edinburg, TX area.
About the RoleWe are looking for a skilled nurse practitioner or physician assistant to provide high-quality care to our patients in the comfort of their own homes. As a member of our team, you will work closely with a multi-disciplinary team of healthcare providers to deliver comprehensive care to our patients.
Responsibilities- Provide primary care to patients in their homes, including diagnosing and treating acute and chronic conditions
- Manage patient care in a home setting, with occasional visits to an office or other facility as needed
- Collaborate with other healthcare professionals to develop and implement patient care plans
- Prescribe medications and other interventions as needed
- Facilitate patient and family decision-making regarding treatment options and end-of-life care

- 160 hours of PTO and 40 hours of CME per year
- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA)
- Current licensure through the Texas Board of Nurse Examiners or the Texas State Medical Board
- Valid DEA license or ability to obtain one prior to employment
- 1+ year of clinical experience as a nurse practitioner or physician assistant
- Comfortable managing a primarily Medicare-aged patient population
- Home health experience
- Bilingual (English/Spanish) fluency
- Working understanding and support of a managed care environment
