Job Summary

We are seeking a skilled and compassionate RN or LPN to join our team as a Home Health Nurse. As a Home Health Nurse, you will be responsible for providing high-quality patient care in the comfort of their own homes. This is a challenging and rewarding role that requires strong clinical skills, excellent communication, and a commitment to delivering exceptional patient outcomes.

Key Responsibilities
  • Assess and plan patient care to meet their individual needs and goals.
  • Implement and evaluate patient care plans in collaboration with the healthcare team.
  • Provide education and support to patients and their families to promote health and well-being.
  • Collaborate with other healthcare professionals to ensure seamless care coordination.
  • Document patient care accurately and thoroughly to maintain high-quality records.
Requirements
  • Current RN or LPN license in the state where you practice.
  • Current PPD or Chest X-Ray to ensure you are free from tuberculosis.
  • Current BLS card to demonstrate your ability to respond to emergencies.
  • One year prior nursing experience preferred, but not required.
Skills and Qualifications
  • Strong clinical skills and ability to work independently.
  • Excellent communication and interpersonal skills to work effectively with patients, families, and healthcare teams.
  • Ability to work in a fast-paced environment and prioritize tasks efficiently.
  • Basic computer skills to document patient care and communicate with the healthcare team.
