We are seeking a skilled Nurse Care Manager to join our team at ResCare, Inc. As a Nurse Care Manager, you will be responsible for providing high-quality nursing care to our clients in a variety of settings.
Key Responsibilities- Assess health problems and needs of clients, develop and implement nursing care plans, and maintain accurate medical records.
- Administer nursing care, advise clients and staff on health maintenance and disease prevention, and provide case management.
- Review daily appointment needs, monitor staff, and ensure coverage.
- Monitor health status, vitals, and seizure disorders, and address acute and ongoing health issues.
- Attend Individualized Plan Progress (IPP) meetings, review medication administration records, and provide on-call service.
- Perform PRN assessments, review IPP schedules, and submit assessments two weeks prior to IPP meetings.
- File healthcare documents, review medication storage, and order medications with remaining stock.
- Complete monthly nursing summaries and reconcile medications.
- Develop and update medication support plans.
- Degree from an accredited school of nursing or current and valid Registered Nurse license.
- Valid driver's license from the state of residence.
- At least one year of nursing experience, with supervisory experience preferred.
- Minimal travel may be required to assigned operations or locations.
