Job Title: Registered Nurse - Hospice Care

VNA Health Group is seeking a compassionate and skilled Registered Nurse to join our dedicated Hospice Team in Ocean County, New Jersey.

About the Role:

As a Registered Nurse on our Hospice Team, you will provide high-quality patient care to individuals with life-threatening and life-limiting illnesses. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans, ensuring the best possible outcomes for our patients and their families.

Responsibilities:
  • Assess and evaluate patient needs, developing and implementing effective care plans
  • Provide direct patient care, including medication management and wound care
  • Collaborate with patients, families, and healthcare teams to ensure comprehensive care
  • Document patient progress and outcomes
  • Participate in ongoing education and training to stay current with best practices
Requirements:
  • Current RN licensure in New Jersey
  • 1+ year of experience in hospice or palliative care
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
