The successful candidate will be responsible for providing support services to families with children aged 0-5 years, including home visits, family-centered assessments, and goal planning.
Key Responsibilities- Provide support services to families with children aged 0-5 years, including home visits, family-centered assessments, and goal planning.
- Connect families to outside resources and referrals, and provide coordination and advocacy for services needed to assist families in working towards their goals.
- Manage a caseload of 15-20 clients and their families, ensuring project objectives and deliverables are met and comply with GPTLHB and GP-TMIECHV policies and procedures.
- Completion of a formal training program or an associate's degree and one (1) year of relevant experience; or a high school diploma or GED and three (3) years of relevant experience.
- Familiarity and/or experience working with American Indian populations and respect for and knowledge of traditional, cultural, and spiritual practices of diverse American Indian communities.
- A competitive salary of $65,000 - $80,000 per year.
