LHC Group is seeking a skilled Home Health Physical Therapist Assistant to join our team in Passaic, New Jersey.

Job Summary

We are looking for a dedicated and compassionate Physical Therapist Assistant to provide high-quality care to our patients in the comfort of their own homes. As a Home Health Physical Therapist Assistant, you will work closely with our team of healthcare professionals to develop and implement personalized treatment plans that promote optimal patient outcomes.

Responsibilities
  • Provide exercise instruction and patient education to improve patient well-being and enhance their quality of life.
  • Work closely with the supervising Physical Therapist to develop and implement treatment plans that meet the unique needs of each patient.
  • Conduct home visits to assess patient needs and provide treatment as prescribed by the Physical Therapist.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
Requirements
  • Current licensure, registration, or certification to assist in the practice of physical therapy in the state of New Jersey.
  • Current CPR certification.
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
