Job Summary

We are seeking a skilled LPN to join our home health team in West St. Paul. As a LPN, you will provide high-quality nursing care to clients in their homes, working closely with our interdisciplinary team to ensure the best possible outcomes.

Key Responsibilities
  • Direct Client Care: Provide preventative and rehabilitative nursing procedures as directed by the Registered Nurse, observing signs and symptoms and reporting to the physician reactions to treatments, including drugs, and changes in the client's physical or emotional condition.
  • Client Records and Billing: Accurately document services rendered utilizing agency EMR, demonstrating understanding and knowledge of all payer sources and authorization required, and completing and documenting supervisory visits with the HHA per regulations.
  • Communication: Effectively communicate with clients, families, referral sources, physicians, and the interdisciplinary team, demonstrating exceptional verbal and written communication skills.
  • Other Duties: Perform other duties as assigned by the Regional Clinical Manager.
Requirements
  • Must be licensed as a Practical Nurse in the state of Minnesota.
  • Must obtain CPR certification within 6 months from date of hire.
  • Must have a valid driver's license and use of an insured personal vehicle.
  • Must complete and pass a state required background study.
  • Able to communicate verbally and in writing to the extent required by the position.
  • Able to physically perform the duties required by the position.
  • Required to have the ability to read, write and speak English well enough to communicate with staff, clients and families.
