Under the guidance of a collaborative physician, physician site leader, and practice manager, the Nurse Practitioner (NP) is responsible for providing comprehensive medical care to patients within a defined scope of practice.
This scope of practice outlines the range of clinical modalities and protocols established in collaboration with the supervising physician.
Within the outpatient care environment, the NP conducts physical examinations; orders diagnostic and therapeutic interventions:such as laboratory tests, imaging studies, EKGs, etc.; performs therapeutic procedures including injections, immunizations, suturing, wound care, and infection management; prescribes medications; and educates patients and their families on effective healthcare practices, illness prevention, and home care strategies; while meticulously documenting progress notes and orders in the patient chart.
These responsibilities are executed within the established scope of practice to optimize patient outcomes.The NP must be credentialed as a mid-level provider by the same healthcare facilities where the supervising physician has admitting privileges.
Definitions:
Collaborative Agreement:
A comprehensive document detailing the qualifications required for the NP to maintain their role, including continuing education requirements, supervision by the collaborative physician, autonomy in medical decision-making, prescriptive authority, and the overall relationship with other practice members.
Collaborative Physician:
A Medical Doctor overseeing the NP's practice.
Scope of Practice:
A detailed outline of the medical practices performed by the NP based on their professional license, competence, education, and training, including the range of patients treated, procedures performed, prescriptive authority, screenings, and physician-directed procedures.
NOTE:
The collaborative agreement and scope of practice must accompany this Job Description.
The Opportunities Ahead:
Depending on practice site requirements, scope of practice, and the NP's education, training, and experience, the incumbent may not perform all key responsibilities listed below.
Patient Examinations and Treatment
Conducts and documents thorough physical examinations to assess specific health concerns, including a comprehensive history and physical assessment. Analyzes and interprets clinical data, formulates presenting problems and their causes, and develops care plans for resolution. Evaluates and manages acute minor illnesses and injuries in accordance with established practice protocols. May assist the physician with procedures or, based on scope of practice, conduct invasive or non-invasive procedures under supervision.
Orders Diagnostic and Therapeutic Modalities
In collaboration with the supervising physician, orders laboratory tests, imaging studies, and/or other non-invasive diagnostic procedures. Initiates referrals to appropriate healthcare professionals for issues that exceed the NP's scope of practice.
Prescribes Medication
Prescribes medications in accordance with the established scope of practice under the oversight of the collaborative physician and mid-level practitioner controlled substance license.
Patient and Family Education
Instructs patients and their families on effective healthcare practices, appropriate illness prevention strategies, and home care in alignment with practice protocols and the patient's physical condition.
Hospital and Nursing Home Daily Rounds
Depending on scope of practice and facility requirements, may conduct daily rounds of hospitalized patients under the supervision and guidance of the collaborative physician.
May be expected to contribute to care discussions and provide consultation to patients, physicians, nurses, and other healthcare professionals to enhance clinical care.
Provides updates to the supervising physician.Patient Documentation
Ensures accurate documentation of patient-related information in the medical record, including discharge summaries and instructions.
Performs all other duties/responsibilities as assigned in accordance with the scope of practice.
Qualifications:
The Experience You Bring:
Graduation from an accredited Nurse Practitioner program with a Master's Degree. National certification as a Nurse Practitioner in the relevant specialty is required (ANCC). Current state licensure in good standing as an Advanced Practice Nurse and Registered Nurse. Current CPR certification must be maintained (BLS). Depending on prescriptive authority, an active Mid-level Practitioner Controlled Substance License and DEA License may be necessary.
A minimum of 1-2 years of experience working as a Nurse Practitioner in a relevant specialty.
Interest in the geriatric patient population.
Demonstrated ability to provide exceptional service to patients, with a friendly and approachable demeanor, effectively resolving patient concerns, and understanding customer service principles.
Able to adapt treatment, instruction, and education to meet the patient's age and cultural background.
