Job Summary

Evaluate and assess the nutritional needs of patients, developing and implementing personalized plans of care based on evidence-based practice and diagnosis-related care guidelines.

Accurately identify patients at nutritional risk and complete nutritional assessments using established, evidence-based guidelines.

Collaborate with healthcare teams to provide nutrition education and support, adapting to changes in the work environment and recognizing opportunities for process improvement.

Stay up-to-date with current nutrition publications and research, applying findings to nutrition care practice and patient education materials.

Key Responsibilities:

  • Evaluate patient outcomes and progress toward achieving nutrition goals, revising intervention and education plans as needed.
  • Develop and implement patient nutrition care protocols and educational materials based on evidence-based practice.
  • Participate in performance improvement initiatives and contribute to the development of department or system process improvements.
  • Assess nutrition education needs and provide individualized education to patients and families.

Requirements:

  • Master's degree in nutrition or a related field, as required by the Commission on Dietetic Registration.
  • Current registration by the Commission on Dietetic Registration as a Registered Dietitian.
