Job Description:

We are seeking a skilled Speech Language Pathologist to join our team at Supplemental Health Care. As a Speech Language Pathologist, you will work with patients in a home health setting, providing personalized care and support to help them achieve their goals.

Responsibilities:

  • Provide speech and language therapy services to patients in a home health setting
  • Develop and implement individualized treatment plans to meet the unique needs of each patient
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Document patient progress and maintain accurate records

Requirements:

  • Graduate of an accredited SLP program
  • Licensed as a Speech Language Pathologist in Ohio
  • CPR Certified or ability to obtain
  • Previous experience with Computer Charting
  • 6 months of previous Home Health Care experience
