About the Role

In this role, you will work closely with patients, healthcare providers, and community agencies to promote health equity and provide supportive services to individuals and families. Your primary goal will be to help bridge the gap between healthcare systems and community resources, facilitating access to essential services and information.

Responsibilities
  • Establish trusting relationships with patients and their families, providing support, encouragement, and feedback.
  • Coach patients in effective management of chronic health conditions and self-care.
  • Convey the purposes and services of programs to user populations, highlighting the impact on health outcomes.
  • Assist patients in understanding care plans and instructions.
  • Visit patients in their homes to assess needs, meet with family members or caregivers, and document activities.
  • Document service plans, results, and patient interactions in accordance with established policies and procedures.
  • Act as a liaison between patients/families and community service agencies, facilitating access to resources and services.
Requirements
  • Must be 18 years or older.
  • Live within 30-45 minutes from Milwaukie, OR.
  • High school diploma or GED required.
  • U.S. Citizen or Permanent Resident/Green Card holder (non-citizens/Visa holders not eligible).
  • Familiarity with Microsoft Office Suite beneficial.
  • Pass a national criminal background check successfully.
Benefits
  • Competitive salary of $15.00 per hour ($31,200 annualized), paid semi-monthly.
  • Stipend of $7,500 for the duration of the program.
