POSITION TITLE: Licensed Physical Therapist

DEPARTMENT: Clinical Services

REPORTS TO: Director of Rehabilitation

POSITION SUMMARY:

At Embrace Rehab Healthcare, we prioritize the well-being of our patients by fostering a compassionate and respectful environment. The Licensed Physical Therapist is responsible for assessing and treating patients based on physician referrals, utilizing therapeutic techniques aimed at enhancing physical function, independence, safety, and overall quality of life.

The Physical Therapist plays a vital role in ensuring the delivery of high-quality and efficient care throughout the treatment process. This includes supervising Physical Therapist Assistants and coordinating care effectively.

Key Responsibilities:

  • Respond to and initiate patient referrals.
  • Analyze available patient data to inform assessment and treatment strategies.
  • Create and execute personalized treatment plans tailored to patient needs, incorporating achievable functional goals.
  • Maintain clear and concise documentation of treatment progress, including discharge summaries in the required format.
  • Guide Assistants in treatment methodologies and oversee their work with patients.
  • Educate patients and their families about program services and treatment protocols, including potential risks.
  • Instruct patients and caregivers in exercises and therapeutic techniques to be continued post-discharge.
  • Engage in continuing education to uphold clinical competency and professional development.
  • Contribute to quality improvement initiatives by responding to audits, surveys, and regulatory requirements.
  • Evaluate the competency of Physical Therapy Assistants annually.
  • Perform additional duties as required.

ESSENTIAL PHYSICAL REQUIREMENTS:

  • Ability to move throughout the work environment consistently.
  • Capability to lift over 50 pounds with assistance, depending on patient needs.
  • Ability to sit and walk for extended periods.
  • Willingness to travel as necessary for patient care.
  • Ability to bend, squat, push, stoop, and pull as required for patient care activities.
  • Meet health requirements as outlined by facility policies, including physical examinations.

WORKING CONDITIONS:

Potential exposure to blood, body fluids, infectious diseases, and hazardous materials. Adherence to safety policies and health regulations is mandatory.
