**Job Title:** Care Coordinator
**About the Role:**
We are seeking experienced individuals to join our team as Care Coordinators. As a Care Coordinator, you will work closely with children, youth, and their families to link them to needed care and services. Our service area includes multiple counties in Ohio.
**Responsibilities:**
* Provide community-based services to children and youth with behavioral or developmental disabilities and their families
* Work with community partners to identify and connect individuals to necessary resources
* Develop and implement individualized care plans to meet the unique needs of each child and family
* Collaborate with healthcare providers, educators, and other professionals to ensure comprehensive care
**Requirements:**
* High school diploma and 3 years of experience, or an associate's or bachelor's degree and 2 years of experience, or a master's degree and 1 year of experience
* Experience in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a related field
* Expertise in family systems, community systems, case management, child and family counseling, or child development
* Proficiency in computer systems and a valid driver's license
