Nurse Case Manager

We are seeking a skilled Registered Nurse Case Manager to join our team at Interim HealthCare of New Hampshire. As a key member of our healthcare team, you will be responsible for managing a defined caseload of patients, providing direct patient care, and delivering comprehensive nursing services to individuals in the community.
Job Responsibilities:- Manage a caseload of patients, providing direct care and support to ensure optimal health outcomes.
- Develop and implement individualized care plans, taking into account patient needs, preferences, and goals.
- Collaborate with healthcare professionals, families, and caregivers to ensure seamless care coordination.
- Conduct thorough assessments, identify patient needs, and develop strategies to address them.
- Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle choices.
- Document patient progress, outcomes, and any changes to care plans.
- Valid nursing license in the state of New Hampshire.
- 2+ years of experience in skilled care, preferably with a focus on wound care.
- 1+ year of experience in nursing supervision or leadership.
- Current CPR certification and reliable transportation.
