Qualifications for MDS Care Plan Coordinator LPN/RN:

  • Current RN License in Colorado or Compact State
  • CPR Certification Required
  • Preferred Minimum of One Year Experience as a Registered Nurse
  • MDS Experience is Highly Desirable

Responsibilities of MDS Care Plan Coordinator LPN/RN:

  • Facilitate and oversee the accurate and timely completion and submission of interdisciplinary MDS Assessments and Care Plans
  • Adhere to State and Federal Regulations
  • Comply with Facility Policies and Procedures
  • Perform Additional Duties as Assigned
