Job Summary

We are seeking a compassionate and reliable Home Health Aide to provide personalized support to our clients in the comfort of their own homes. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our clients and their families.

Responsibilities
  • Provide direct and indirect personal care to clients in the home setting
  • Establish and maintain effective communication and a professional relationship with clients, family members, and co-workers
  • Perform duties as assigned on personal care service plan
  • Help client with all personal care functions as needed
  • Help keep the client's environment clean and organized
  • Remind client to take self-administered medications
Qualifications
  • Must have Reliable Transportation
  • Strong written and oral communication skills in English

  • Providing smooth transitions home from hospitals or rehabilitation facilities after surgeries
  • Ensuring clear, prompt communication to families and other health care providers - through real-time access to our care notes, and our Care Advocate updates
  • Helping families care for their loved ones while minding all of the details on causes for admissions to hospitals, and having caregivers specifically matched to our clients particular needs

Our caregivers are trained to identify and escalate concerns related to the top causes for trips to the hospital, ensuring our clients receive the best possible care.

We look for caregivers with the highest standards of care and a track record of dependability.
