Emergency Department Clinical Care Coordinator Lead

At Mary Washington Healthcare, we are seeking an experienced Emergency Department Clinical Care Coordinator Lead to join our team. As a key member of our healthcare team, this role is responsible for ensuring the efficient management of admission flow in our emergency department.
About the RoleThis position serves as a crucial link between the emergency department and other hospital departments, ensuring seamless patient progression and care coordination. The ideal candidate will possess strong clinical knowledge, excellent communication skills, and the ability to work independently in a fast-paced environment.
Key Responsibilities- Assess patients' clinical presentation against Milliman Care Guidelines to determine admission status, level of care, and target length of stay (LOS).
- Collaborate with emergency department and hospitalist providers to evaluate appropriateness of potential admissions and identify alternatives for patients not meeting admission criteria.
- Evaluate patient discharge needs and develop comprehensive discharge plans that address patient requirements, barriers, and readmission risk.
- Communicate plans of care and discharge plans to patients and their families in coordination with primary care teams.
To be successful in this role, you will need:
- A minimum of three years recent acute care clinical experience.
- A Bachelor of Science in Nursing degree (preferred).
- The ability to work independently, manage time effectively, and prioritize multiple tasks to achieve goals.
- Strong critical thinking and problem-solving skills, with the ability to recognize situations requiring immediate intervention.
- Excellent written and verbal communication skills.
We welcome candidates with:
- One year supervisory experience.
- Clinical experience in critical care or emergency nursing.
- Utilization review experience.
We offer a competitive salary range of $75,000 - $90,000 per annum, depending on experience. Additionally, we provide a comprehensive benefits package, including medical, dental, and vision insurance, 401(k) matching, and paid time off.
