Physical Therapist

We are seeking a skilled Physical Therapist to join our team at Elderwood at Grand Island. As a Physical Therapist, you will play a vital role in ensuring the health and well-being of our residents by providing comprehensive and quality therapy services.
Responsibilities:- Initiate skilled physical therapy treatments in accordance with physician's orders and maintain required contact with physician.
- Maintain written evaluations, progress notes, and discharge summaries for individuals in physical therapy program.
- Confer with other healthcare staff regarding special needs of individuals to enhance rehabilitation or maintenance of function.
- Provide input for meetings and attend during absences of the Director Physical Therapy.
- Maintain statistical records of individuals' treatments.
- Assist with in-service training programs upon request.
- Record and update individual's plan of care in the Inter-disciplinary Kardex.
- Submit requisition for supplies needed in the treatment to Director Physical Therapy.
- Assume all duties of the Director Physical Therapy during absences, including supervision of Physical Therapy Aides and responsibility for all necessary documentation.
- Graduation from an approved school of physical therapy required.
- State license to practice physical therapy required.
- Two years' experience in physical therapy preferred.
- Ability to direct others necessary.
- This position requires regular interaction with residents, coworkers, visitors, and/or supervisors.
