Job Summary

We are seeking an experienced Registered Nurse (RN) Case Manager to join our team at Heart of Hospice, LLC. As an RN Case Manager, you will be responsible for evaluating patient eligibility for hospice services, assessing physical and psychosocial needs, and developing personalized care plans. Your primary focus will be on delivering high-quality patient care, building trusting relationships with patients and their families, and collaborating with healthcare professionals to ensure seamless coordination of care.


Responsibilities
  • Evaluate patient eligibility for hospice services and assess physical and psychosocial needs.
  • Develop and implement individualized care plans that address the unique needs of each patient.
  • Collaborate with healthcare professionals to ensure comprehensive care coordination.
  • Document clinical notes and update care plans as needed.
  • Maintain accurate records and communicate effectively with patients, families, and healthcare teams.

Requirements

To be successful in this role, you must have:

  • One year of clinical experience.
  • Current RN licensure in the state of practice.
  • Current CPR Certification.
  • A valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
