Key Responsibilities:
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Observe, record, and report patient reactions, changes, and progress to the attending physician and other healthcare professionals.
  • Develop and implement individualized plans of care that promote optimal patient outcomes.
  • Collaborate with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Communicate effectively with patients, families, and healthcare professionals to ensure understanding and compliance with treatment plans.
Requirements:
  • Current licensure as a Physical Therapist in the state of employment.
  • Valid driver's license and auto insurance in your name as a driver.
  • Ability to meet the physical demands of the job, including lifting, bending, and standing for extended periods.
