Travel Nurse Practitioner Home Health Assessment Position

We break through traditional barriers to care by meeting our members where they are. Our clinicians help older adults and other at-risk individuals enjoy improved quality of care, experience enhanced health outcomes, and identify chronic conditions that may otherwise go undiagnosed.
The RoleYou will make a direct impact on the health and well-being of others by conducting Adult/Geriatric Assessments, medical history reviews, diagnoses, treatments, health education, physician referrals, case management referrals, follow-ups, and clear documentation according to Matrix guidelines and protocols.
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and any appropriate ancillary medical personnel as needed.
- Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up support.
- Maintain relationships with a nationwide network of advanced practice providers.
Services may be provided in various venues, including home visits, skilled facility visits, and telehealth visits.
Requirements- Master's degree or equivalent experience and satisfactory completion of NP licensure required.
- Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.
- Current BLS, ACLS, or CPR certification.
- Minimum 1 year experience as a Nurse Practitioner.
- Strong computer skills and familiarity with Employee Health/Medical Record software.
- Excellent verbal and written communication skills with patients, clients, and colleagues.
- Ability to travel and have a valid state driver's license, proof of adequate automobile insurance coverage for the state of residence.
PAY: $105.00 or $110.00 per completed assessment, depending on county, with flexible scheduling options available.
Benefits include mileage reimbursement for 20 or more completed assessments per month and no-show payment for 10 or more booked assessments per month.
