We are seeking a skilled Nurse Care Coordinator to join our team at Corewell Health, Southwestern Medical Clinic. This is a full-time position working 40 hours per week, Monday through Friday, from 8:00 a.m. to 4:30 p.m.

About the Role:

The successful candidate will be responsible for providing high-quality patient care and ensuring seamless transitions between healthcare services. As a Nurse Care Coordinator, you will work closely with patients, families, and healthcare teams to develop individualized care plans, addressing physical, emotional, and social needs. Your role will involve assessing patient needs, coordinating care, and communicating effectively with various stakeholders to ensure optimal patient outcomes.

Responsibilities:

  • Assess patient needs and develop personalized care plans in collaboration with multidisciplinary teams.
  • Coordinate care activities, including scheduling appointments, testing, and referrals, to ensure timely and effective delivery of services.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and shared understanding of care plans.
  • Monitor patient progress, adjusting care plans as needed to optimize outcomes.
  • Collaborate with community resources and organizations to provide comprehensive support and services to patients and families.

Requirements:

  • A minimum of an Associate's Degree in Nursing (ADN) or Bachelor's Degree in Nursing (BSN) required; BSN preferred.
  • Current licensure as a Registered Nurse (RN) in the State of Michigan required.
  • Basic Life Support (BLS) certification required within 90 days of hire; Advanced Cardiovascular Life Support (ACLS) certification may be required depending on unit assignments.
  • Excellent communication, interpersonal, and organizational skills necessary to collaborate effectively with patients, families, and healthcare teams.
