Job Summary
We are looking for a skilled RN with experience in pediatric care to provide high-quality patient care in a transitional care setting. The ideal candidate will have a strong background in pediatrics, with a focus on transitional care and a passion for delivering exceptional patient outcomes.
Responsibilities
- Provide direct patient care to pediatric patients in a transitional care setting
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with the interdisciplinary team to ensure seamless transitions of care
- Monitor patient progress and adjust care plans as needed
- Communicate effectively with patients, families, and healthcare team members
Requirements
- Current RN licensure in the state of Illinois
- 2+ years of experience in pediatric care, with a focus on transitional care
- Trach experience, Vent experience, Gtube experience
- Covid vaccination
- PICU experience preferred
