RN Care Coordinator Job Summary

We are seeking a skilled RN Care Coordinator to join our team at LifeBridge Health. As a RN Care Coordinator, you will play a critical role in managing individualized patient-centered care through the use of the Nursing Process and the principles of Primary Nursing.

Key Responsibilities:

  • Manage a panel of patients with complex chronic conditions and/or rising risk indicators.
  • Perform comprehensive needs assessments and develop plans of care to improve health outcomes, reduce healthcare costs, and enhance patient experience.
  • Coordinate care transitions, referrals, and appointments to ensure timely access to services.
  • Facilitate patient and family education, medication reconciliation, and adherence.
  • Evaluate the effectiveness of care plans and make adjustments as needed.

Requirements:

  • Graduate of an accredited school of nursing with a Bachelor's Degree required, Master's Degree preferred.
  • Current/Valid Registered Nurse License in the State of Maryland (or Compact State as applicable).
  • Case/Care management certified or knowledge of national care management standards and community resources a plus.
  • 3-5 years demonstrated proficiency in acute care nursing, knowledge, and skills.
  • High level verbal and communication skills, organizational skills, analytical, data management, and computer skills.
