Job Summary:The Dialysis Outpatient Registered Nurse will be responsible for assessing, planning, and implementing individualized patient care plans, as well as collaborating with the interdisciplinary team to achieve optimal patient outcomes. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
Responsibilities:- Assess and evaluate patients' physical and emotional needs, developing and implementing individualized care plans to meet those needs.
- Administer medications and treatments as prescribed, monitoring patients' responses and adjusting care plans as necessary.
- Collaborate with the interdisciplinary team to develop and implement patient care plans, ensuring seamless communication and coordination of care.
- Provide education and support to patients and their families, promoting self-care and independence.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.
- Current RN licensure in the state of employment.
- Minimum of 9 months of experience as a Registered Nurse, with a focus on chronic/acute hemodialysis.
- Successful completion of Fresenius Medical Center's education and training requirements.
