Job Summary

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our Home Health team as a Home Health Registered Nurse (RN). As a Home Health Registered Nurse (RN), you will provide high-quality patient care in the comfort of their own homes, working closely with a multidisciplinary team to ensure seamless care transitions.

Key Responsibilities
  • Provide comprehensive home-based care to patients with various medical conditions, including chronic diseases, injuries, and illnesses.
  • Conduct thorough assessments, develop and implement individualized care plans, and coordinate with healthcare providers to ensure optimal patient outcomes.
  • Collaborate with a team of healthcare professionals, including physicians, LPNs, CNAs, and aides, to deliver patient-centered care.
  • Document patient progress, report changes to physicians, and educate patients and their families on care plans, medication management, and home safety.
  • Ensure timely and effective discharge planning, coordinating with healthcare providers and community resources as needed.
Requirements
  • Graduate of an accredited nursing program and active RN license in Georgia.
  • Minimum of 2 years of nursing experience in home healthcare, with a strong background in clinical proficiency and patient assessment.
  • Experience with HCHB and OASIS charting, as well as CPR certification and negative ppd screening or chest x-ray.
  • Valid driver's license, reliable transportation, and auto insurance.
  • Knowledge of state and federal home health regulations, as well as strong communication and interpersonal skills.
