About This Role

We are seeking a skilled and compassionate Home Health RN Field Nurse to join our team at Stonerise Home Health Barboursville. As a key member of our care team, you will play a vital role in delivering high-quality patient care and promoting independence and dignity in the comfort of patients' own homes.

Key Responsibilities
  • Assess and Plan Care
    • Conduct thorough initial assessments of patients and families to determine home care needs
    • Develop and implement comprehensive care plans that address patients' physical, emotional, and social needs
  • Coordinate Care Services
    • Supervise and direct Licensed Practical Nurses (LPNs) and Home Health Aides in implementing patient care plans
    • Collaborate with healthcare providers to ensure seamless care transitions and optimal patient outcomes
  • Provide Education and Support
    • Develop and implement patient education plans to promote self-care and independence
    • Provide emotional support and guidance to patients and families during times of need
  • Monitor and Evaluate Care
    • Regularly evaluate patient progress and adjust care plans as needed
    • Document patient outcomes and care provided in accordance with regulatory requirements
Requirements
  • Graduate of an accredited school of nursing
  • Active RN licensure
  • One to two years of recent acute care experience in an institutional setting
  • Bachelor's degree with one year of home health care experience preferred
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
