Our team of clinicians travel to various patient homes throughout the day, ensuring optimal care and aiding in the transition from hospital to home. We focus on post-hospitalization recovery, rehabilitation, and disease management, serving a diverse group of patients with unique needs. Our clinicians promote independence, educate patients, and ensure a smooth recovery process.
Responsibilities:
- Provide high-quality patient care in the home setting
- Develop and implement individualized care plans
- Collaborate with interdisciplinary teams to ensure comprehensive care
- Conduct wound care, post-operative follow-ups, and therapy sessions
Requirements:
- Current Oklahoma RN licensure
- Valid driver's license
- CPR certification
- At least one year of Home Health/Hospice experience as a RN
