About the Position

As a Nurse Practitioner on our PRN/Part Time team, you will be responsible for conducting home risk assessments, reviewing medical history, and providing health education to our patients. You will also work collaboratively with our team of clinicians and healthcare professionals to develop and implement care plans that meet the unique needs of each patient.

We are looking for a skilled and compassionate Nurse Practitioner who is able to work independently and as part of a team. You will have the opportunity to work in a variety of settings, including home visits, skilled facilities, and telehealth visits.

Responsibilities
  • Conduct home risk assessments and review medical history
  • Provide health education to patients and their families
  • Work collaboratively with our team of clinicians and healthcare professionals to develop and implement care plans

Qualifications
  • Master's degree in nursing or related field
  • Current RN and NP licensure in state of practice
  • Board certification by AANP, ANCC, or AACCN
  • 1 year of experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
