Position Overview:
The Patient Care Scheduling Specialist at CorsoCare is responsible for effectively managing the coordination of care within our hospice program.

Key Responsibilities:
- Oversee the assignment of various healthcare professionals to patients, including nursing staff, social workers, spiritual advisors, and other essential disciplines as determined by the Interdisciplinary Group (IDG) and tailored care plans.
- Ensure seamless scheduling among multiple providers to optimize patient care, enhance service efficiency, and adhere to physician directives.

Qualifications:
- A minimum of three (3) years of relevant experience in hospice, home health, or interdisciplinary team settings.
- Proficient understanding of Medicare, Medicaid, and insurance regulations pertinent to hospice care.
- Familiarity with electronic data entry systems and report generation.
- Competence in Microsoft Outlook 365, Microsoft Teams, and Microsoft Word/Excel, along with professional phone etiquette and EMR documentation.
- Strong commitment to customer service, teamwork, and organizational skills, with an emphasis on detail orientation and flexibility.
- Excellent verbal and written communication abilities, with a knack for multitasking and maintaining accuracy.

Essential Duties:
- Coordinate patient scheduling in collaboration with the Clinical Director/Manager.
- Conduct introductory calls to new patients to facilitate their transition into care.
- Provide insights on hospice insurance benefits to patients and their families.
- Assist in the initial setup of necessary equipment, supplies, and medications through effective communication with patients and families.
- Maintain the master schedule for the care team and ensure visit verification.
- Participate in daily meetings to address any changes in patient care needs or scheduling conflicts.
- Manage the Face-to-Face encounter schedule for hospice physicians or CRNPs.
- Collaborate with the Clinical Educator to coordinate training schedules for new staff members.
- Update staff schedules regularly and generate reports on visits and caseloads.
- Verify daily visit activities and address any discrepancies with the Clinical Director/Manager.
- Collaborate with the Inpatient Manager to manage bed capacity and coordinate patient admissions.
- Adapt schedules as necessary in response to staff absences or increased service demands.
- Communicate schedule changes to patients and families in a professional manner.
- Ensure compliance with all relevant regulations and maintain confidentiality of patient information.
