Job Summary:

Seeking a skilled Nurse Practitioner or Physician Assistant to provide primary care services in a post-acute care setting. The ideal candidate will have experience in managing chronic disease, diagnosing illnesses, and implementing plans of care.

**Key Responsibilities:**

* Perform primary care in a post-acute setting for short-term and long-term patients
* Manage chronic disease, diagnose illnesses, and implement plans of care
* Order and interpret lab results
* Prescribe and monitor medications
* Consult with physicians and coordinate patient care with other healthcare practitioners
* Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
* Build relationships with patients, patients' family members, and facility staff to facilitate effective treatment delivery
* Promote healthy lifestyle changes

**Requirements:**

* Minimum of 1 year experience in primary care
* Post-acute care experience a plus, but not required
* Active and unrestricted Washington NP or PA license required
* Active and unrestricted DEA required

**Compensation and Benefits:**

* The compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential
* Compensation range $115,000 - $167,000
