Job Title: RN PRN Position

We are seeking an experienced RN to join our team at Good Shepherd Hospice, serving patients and their families in Duncan and surrounding communities.

About the Role:

The Hospice RN provides nursing assessment, planning, and care to maximize the comfort and health of patients and families in accordance with the Interdisciplinary Plan of Care.

Key Responsibilities:
  • Receives patient assignments from the Patient Care Manager or Case Manager and initiates communication with attending physicians, hospice staff members, and other agencies as needed.
  • Cultivates communication and bereavement skills to provide optimal care and support to patients and families.
  • Maintains regular communication with the attending physician concerning patient/family care and ensures accurate and up-to-date patient records.
  • Contacts families of new patient assignments and maintains regular communication as requested or desired by family.
  • Obtains data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporates that data into the Plan of Care.
  • Provides education and support to patients and caregivers on medication administration, personal care, oral hygiene, lifting, moving, ambulation, and exercise.
  • Attends Hospice team meetings and other patient conferences as deemed necessary by the Patient Care Manager.
  • Provides appropriate support at the time of death and shares in providing 24-hour, seven days a week coverage to patients and families.
Requirements:
  • Registered Nurse currently licensed in the state in which the care is provided.
  • Hospice experience preferred.
  • Must be of high integrity, including maintenance of confidential information, including all patient records.
  • Works as a team player, following directions, and adhering to defined care plans.
  • Demonstrated commitment to the hospice philosophy of care.
  • Strong organizational and self-management skills.
  • Critical thinker with good judgment.
  • Valid Driver's License.
