Key Responsibilities:
The ideal candidate will:
- Exhibit a thorough understanding of hospice principles and effectively communicate these to patients and families.
- Manage the plan of care from initial referral through to discharge, ensuring compliance with regulations and agency standards.
- Conduct initial nursing assessments to identify the needs of patients and their families.
- Perform complete physical assessments and gather comprehensive medical histories.
- Collaborate with patients and families to initiate and implement personalized care plans that prioritize patient dignity, autonomy, and comfort.
- Coordinate the delivery of medications, medical equipment, and nursing supplies while employing effective cost-management strategies.
- Anticipate and address after-hours needs as required.
- Administer prescribed medications and treatments, monitor patient responses, and adjust care plans as necessary.
