Position Summary
Our pharmacy division is dedicated to delivering outstanding service and addressing the pharmaceutical needs of various healthcare facilities, including hospitals, rehabilitation centers, and specialized care institutions across the nation.
Key Responsibilities
- Assess physician medication orders for suitability regarding drug selection, dosage, potential interactions, and administration routes. Provide recommendations for therapy modifications to healthcare providers as necessary.
- Maintain a comprehensive patient/resident database that includes relevant medical history, clinical and laboratory data, and current medications, including over-the-counter products.
- Ensure appropriate laboratory tests for monitoring high-risk conditions are ordered, assessed, and communicated with the healthcare team.
- Collaborate with healthcare professionals to deliver clinical and technical support as required.
- Document all interactions with physicians, nursing staff, and patients/residents, including their families, in the medical records.
- Implement policies and procedures to ensure the safe procurement, storage, distribution, and disposal of pharmaceuticals and biological products.
- Acquire and maintain all necessary documentation in compliance with federal and state regulations.
- Notify the Pharmacy Manager of any medication-related incidents.
- Assist facility staff with procedures related to the safe and effective management of pharmaceuticals.
- Perform additional tasks as assigned.
Qualifications
- Bachelor's Degree in Pharmacy; Pharm D is preferred.
- Minimum of one year of experience in long-term care settings.
- Licenses/Certifications: As mandated by regulatory requirements.
- Detail-oriented, goal-driven, and a collaborative team player with strong verbal communication skills.
