Overview

Position Summary:

Central Health is in search of a board-certified physician specializing in Family Medicine or Internal Medicine to enhance our Pre-Operative Assessment and Medical Optimization Programs. This position necessitates a comprehensive understanding of diverse medical conditions, their implications on surgical results, and methodologies for improving patients' health prior to surgical interventions.

The Pre-Operative Assessment initiative will categorize patients based on their risk levels for surgeries or procedures necessitating anesthesia. This role will involve effective communication and collaboration with specialists to ensure thorough evaluations.

Patients identified as high-risk or those with unmanaged chronic health issues during the pre-operative assessment, or recognized during consultations at the Central Health Multi-specialty clinic, will be directed towards our Medical Optimization program for intensive disease management. As a vital member of our interdisciplinary team, this role will require close cooperation with specialists, pharmacists, and dietitians to deliver personalized care that meets each patient's unique needs.

Key Responsibilities:

- Conduct a comprehensive review of the patient's medical history, including previous surgeries, chronic conditions, medication history, allergies, and family history of anesthesia-related complications.

- Perform a detailed physical examination to evaluate the patient's overall health, including vital signs and assessments of cardiovascular, respiratory, neurological, and musculoskeletal systems.

- Review and adjust the patient's current medication regimen to optimize perioperative management, considering potential drug interactions and necessary adjustments.

- Order and interpret relevant laboratory tests and diagnostic studies to evaluate the patient's baseline health and identify any abnormalities affecting perioperative management.

- Assess the patient's perioperative risk based on their medical history, examination findings, and diagnostic results, categorizing them into low, moderate, or high-risk groups to inform management decisions.

- Collaborate with the patient's primary care provider and specialists to enhance the management of chronic conditions prior to surgery, which may involve medication adjustments and lifestyle modifications.

- Educate patients on the significance of preoperative optimization, adherence to medication regimens, and strategies to minimize perioperative risks.

- Address any inquiries or concerns the patient may have regarding their surgical procedure and perioperative care.

- Provide comprehensive education on their conditions, treatment options, and self-management strategies to empower patients in their health journey.

- Communicate findings and recommendations to the surgical team and anesthesiologist to facilitate informed decision-making and coordinated care.

- Ensure meticulous documentation of the preoperative assessment, medical optimization interventions, and communication with the healthcare team in the medical record.

- Develop a plan for postoperative follow-up and continuity of care to monitor recovery and manage chronic conditions beyond the surgical encounter.

- Deliver holistic care by integrating medical, pharmaceutical, and nutritional interventions to enhance health outcomes and quality of life.

- Collaborate effectively with specialists, pharmacists, and dietitians to create and implement tailored care plans for patients, ensuring comprehensive and coordinated care.

- Utilize remote patient monitoring technologies to track progress, identify trends, and modify treatment plans as necessary to optimize patient outcomes.

- Engage in program evaluation and quality improvement initiatives to enhance the effectiveness and efficiency of the medical optimization program.

- Work collaboratively with Central Health's operational and clinical leaders to foster a team-oriented environment.

- Mentor and motivate medical providers under supervision, employing effective performance management processes.

- Champion initiatives that may influence Central Health's operations and the quality of patient care.

- Conduct annual evaluations of direct reports with input from practice leaders and other stakeholders.

- Operate within the scope of credentialing privileges.

- Uphold and exemplify the vision, mission, and values of Central Health.

- Perform additional duties as assigned.

- Proven experience in an integrated delivery environment.

Qualifications

Required Licenses/Certifications:

- Unrestricted license to practice medicine in the State of Texas.

- Board Certification in Family Medicine or Internal Medicine or successful completion of Board Certification examination within two years of employment.

- Current Drug Enforcement Agency (DEA) registration for prescribing medications.

- Current Basic Life Support Certification for Healthcare Providers through the American Heart Association or American Red Cross.

Required Education:

- M.D. or D.O. from an accredited educational institution.

Required Experience:

- Minimum of 1 year in a related field.

Preferred Experience:

- Experience in chronic disease management and perioperative services.
