Job Summary:

We are looking for a skilled and caring LPN to provide high-quality patient care in our community living settings. As a Medical Staff LPN, you will be responsible for planning and managing patient care, interviewing patients, and recording their medical history and physical condition. You will also provide routine care for patients, including medication administration, feedings, and oxygen therapy.

Key Responsibilities:
  • Plan and manage patient care according to each patient's needs
  • Interview patients and record their medical history and physical condition
  • Provide routine care for patients, including medication administration, feedings, and oxygen therapy
  • Monitor clients' vital statistics and report abnormal findings to the case manager and physician
  • Perform medication administration, feedings, oxygen, ostomy care as needed per client's plan of care
  • Complete routines as established by other care professionals, such as Physical Therapist, Occupational Therapist, and Speech Therapist
  • Ensure infection control policies are maintained at all times and report possible communicable diseases or infections
  • Accurately document nursing actions of all care given and communicate with family, case manager, and physicians
