Home Visiting Professional

The Community Action Partnership of Strafford County is seeking a dedicated Child and Family Advocate to join our team. As a key member of our Home Visiting Program, you will work closely with families to identify and address barriers to their success, providing education, support, and resources to promote healthy growth and development of children. This is a dynamic and rewarding role that requires strong communication skills, cultural competence, and a passion for working with families.
Main Responsibilities* Establish and maintain positive relationships with families, building trust and rapport* Conduct assessments to identify family strengths and areas of need* Develop and implement individualized plans to address family goals and objectives* Provide education and support to parents on child development, health, and safety* Facilitate parents' participation in Individual Family Support Plans* Collaborate with other agencies and organizations to ensure families receive necessary resources and services* Maintain accurate records and reports of family interactions and progress* Participate in ongoing training and professional development to enhance knowledge and skills* Demonstrate cultural competence and sensitivity when working with diverse populations* Utilize Solution-Focused Intervention techniques to empower families and promote self-sufficiency* Work collaboratively with the Home Visiting Team to achieve program goals and objectives* Comply with all applicable laws, regulations, and policies related to confidentiality, data protection, and reporting requirements* Participate in regular team meetings and contribute to program planning and evaluation efforts* Perform other duties as assigned by supervisor* Ensure timely completion of tasks and meet deadlines while maintaining high-quality work product* Ability to adapt to changing priorities and work assignments* Possess excellent communication and interpersonal skills, with ability to build relationships with families, colleagues, and community partners* Strong analytical and problem-solving skills, with ability to think critically and creatively* Familiarity with community resources and services, including healthcare, education, and social services* Willingness to learn and grow professionally, with a commitment to ongoing training and development* Demonstrated ability to work independently and as part of a team* Familiarity with Solution-Focused Intervention techniques and principles* Experience working with diverse populations, including low-income families, youth, and adults* Knowledge of community-based programs and services, including Head Start and Early Head Start* Ability to pass a background check and obtain required certifications* Familiarity with electronic record-keeping systems and software used in the field* Valid driver's license and insured vehicle for use on the job
