Job Description for Renal Therapy Coordinator

We are seeking a highly skilled Renal Therapy Coordinator to join our team at DaVita Inc.

About the Role:
  • This is a critical position that requires a strong background in renal therapy, with a focus on coordinating patient care and ensuring seamless transitions between hospital and outpatient settings.
  • The ideal candidate will have excellent organizational and communication skills, as well as the ability to work effectively in a fast-paced environment.

Salary: $80,000 - $110,000 per year

Responsibilities:
  1. Coordinate patient care across multiple settings, ensuring continuity and consistency in treatment plans.
  2. Collaborate with healthcare teams to develop and implement individualized care plans.
  3. Provide education and support to patients and their families to promote healthy lifestyles and disease management.
  4. Maintain accurate and detailed records of patient interactions and outcomes.
Requirements:
  • CurrenRegistered Nurse (RN) license in the state of practice.
  • Associates Degree in Nursing (A.D.N) from accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from accredited diploma program may be substituted for nursing degree.
  • Experience in Intensive Care Unit (ICU), Critical Care Unit (CCU), Emergency Room (ER), or Medical Surgical (Med Surg) preferred.
