Responsibilities
  • Provide comprehensive palliative care to patients with serious illnesses, focusing on pain and symptom management, and advanced care planning.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.
  • Conduct thorough assessments, diagnose, and treat patients with complex medical conditions.
  • Develop and maintain strong relationships with patients, families, and healthcare providers.
  • Stay up-to-date with the latest evidence-based practices and guidelines in palliative care.
Requirements
  • Graduated from an approved school of professional nursing.
  • One year experience as a Nurse Practitioner required in CA.
  • Previous experience in hospice, home health, general medical surgical, or patient population-specific nursing.
  • Minimum 1 year Palliative Care services experience preferred.
  • Demonstrates familiarity with policies of the agency and state(s) and federal regulations pertaining to nursing services.
  • Required Certifications and Licensure: Licensed to practice as a Nurse Practitioner in the state of agency operation with current and unencumbered advanced practice nursing license in state(s) practicing, Active DEA license in the state where care is being provided, Certified by a nationally recognized advanced nursing practice organization, Possess and maintain valid CPR certification while employed in a clinical role, preferred.
