Job Summary

We are seeking a skilled Registered Nurse to join our Home Health team in providing high-quality patient care. As a key member of our team, you will be responsible for delivering and documenting skilled nursing care in accordance with the developed care plan and physicians' orders.

Schedule: We offer flexible weekend visit schedules, allowing you to balance your personal and professional life. Whether you prefer Saturday/Sunday or Friday-Monday shifts, we have options to suit your needs.

Key Responsibilities:
  • Develop comprehensive care plans through collaboration with patients, their families, and healthcare providers.
  • Provide education and training to patients and caregivers on various aspects of health and wellness.
  • Coordinate continuum of care by maintaining open communication with patients' physicians and other healthcare professionals.
  • Deliver hands-on patient care in the comfort of their own homes, utilizing skills such as phone triage, wound care, and infusion management.

Requirements:
  • An active RN license in the state where you reside.
  • Current CPR certification.
Preferred Qualifications:
  • Experience in Medicare Skilled Nursing care.
  • Familiarity with Oasis documentation.
  • A minimum of one year of experience as an RN in a healthcare setting.

Estimated Salary Range: $60,000 - $80,000 per year (dependent on location and experience).
