Key Responsibilities- Provide in-home evaluation, direction, and application of skilled nursing services to complement the patient's overall Plan of Care.
- Ensure the overall patient condition is cared for by providing appropriate education and resources to the patient/caregivers, allowing them to manage their conditions and remain at home.
- Collaborate with the healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.
- Conduct regular assessments and evaluations to ensure the patient's condition is stable and improving.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Graduate of an approved nursing program.
- Minimum one (1) year experience as a professional nurse within the last three (3) years.
- Ability to provide reliable transportation as required by the nature of the position.
- MO RN License.
- BLS must be obtained within 90 days.
- Active Driver's License.
