We are seeking a skilled Licensed Practical Nurse to join our team at Mass General Brigham Healthcare at Home. As a key member of our interdisciplinary team, you will provide high-quality nursing care to patients in various settings in the community.
Responsibilities- Deliver patient care in accordance with MGB Home Care's philosophy, policy, and standards of community health practice.
- Collaborate with RN case managers to provide skilled nursing visits, utilizing thorough and timely electronic documentation and interdisciplinary communications.
- Provide quality nursing care to patients in various settings, including Malden, Melrose, and Everett.
- Utilize effective written, verbal, and electronic communication skills to interact with patients, families, and healthcare team members.
- Work effectively as a member of an interdisciplinary team to provide comprehensive care to patients.
- LPN professional license in Massachusetts.
- Minimum 1 year previous nursing experience in acute care or sub-acute care setting required.
- Home care experience preferred.
- Strong interpersonal skills with ability to work with varied population.
- Ability to work as a member of an interdisciplinary team.
- Strong organizational skills, ability to work independently and adapt to unpredictable circumstances.
