Your Responsibilities:
- Deliver comprehensive hospice nursing care, encompassing personal hygiene assistance, medication administration, and symptom management.
- Collaborate with an interdisciplinary team to develop and implement individualized care plans, addressing the physical, emotional, and spiritual needs of patients and their loved ones.
- Maintain accurate patient records and communicate effectively with physicians and other healthcare professionals.
- Provide support and education to families navigating the complexities of end-of-life care.
- Demonstrate empathy, compassion, and a commitment to providing high-quality care in accordance with hospice principles.
Qualifications:
- Graduate of an accredited professional school of nursing.
- Current Registered Nurse (RN) license.
- Minimum of 2 years of nursing experience, preferably in home health or hospice settings.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Valid driver's license and reliable transportation.
