Job Summary

The Southcentral Foundation (SCF) is seeking a highly skilled and detail-oriented Case Management Support to join our team. As a key member of our Integrated Care Team, you will be responsible for coordinating and managing administrative duties to ensure seamless patient care.

Key Responsibilities
  • Coordinate and manage administrative duties for Primary Care Providers and the wider Integrated Care Team.
  • Provide effective and efficient schedule coordination, verbal and written communication, information research and transfer, and system coordination.
  • Work collaboratively with the core medical team to ensure excellent patient care.
  • Develop and maintain strong relationships with customers and stakeholders.
Requirements
  • High School diploma or equivalent.
  • Administrative support experience or equivalent combination of education and experience.
  • Demonstrated proficiency in administrative support roles.
Additional Requirements for Case Management Support II
  • Two years of Case Management Support experience or demonstrated proficiency as a Case Management Support I.
