Summary

We are hiring a Physical Therapist to join our team in Buckhannon, WV, providing home health care services to patients.

Salary based on full-time employment and max productivity: up to $120,000 per year (based on experience)

Relocation assistance of $5,000 available for those who qualify for full time.

Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.

  • Evaluate the patient's functional status and physical therapy needs. Assist the physician in developing the therapy plan of care.
  • Observe and report the patient's response to treatment and any changes in their condition to the supervising nurse and/or the physician.
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
  • Instruct and inform participating members of the healthcare team, the patient, and their family/caregivers regarding the plan of care, functional limitations, and progress toward goals.


Education and Experience

Licensure Requirements

  • Current Physical Therapy licensure in the state of West Virginia
  • Current CPR certification
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
