We are seeking a skilled Physical Therapist to join our team at Brockton Home Health Care Agency LLC. As a Physical Therapist, you will be responsible for providing high-quality care to our clients in the comfort of their own homes.
Responsibilities:- Make home visits to clients to assess their physical needs and develop personalized care plans.
- Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
- Educate clients and their families on rehabilitative care and activities to promote independent living.
- Accurately document client care management and services provided using an iPad tablet.
- A minimum of one year of recent work experience as a Physical Therapist.
- A minimum of one year of recent Home Health Care work experience.
- Current license to practice as a Physical Therapist in Massachusetts.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
- A supportive and family-owned business environment.
- Comprehensive infection prevention protocol and PPE supplies.
- Closely monitored COVID-19 outbreak protocols.
