Job Title: Travel Nurse RN Case Management

We are seeking a skilled Travel Nurse RN Case Management to join our team at Travel Nurses, Inc. in Edgewood, Kentucky.

About the Job

This is a 13-week travel nursing job with 36 hours per week, offering a flexible 12-hour shift schedule. As a Travel Nurse RN Case Management, you will be responsible for providing high-quality patient care and managing cases in a fast-paced hospital environment.

Responsibilities
  • Provide exceptional patient care and manage cases with a focus on quality and safety
  • Collaborate with healthcare teams to develop and implement effective care plans
  • Conduct thorough assessments and evaluations to identify patient needs
  • Develop and implement individualized care plans to meet patient goals
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
Requirements
  • 1-2 years of recent experience in Case Management
  • Current RN licensure in the state of Kentucky
  • Ability to work flexible 12-hour shifts
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
