The Admissions Physical Therapist plays a crucial role in delivering high-quality patient care by performing initial assessments and developing comprehensive plans of care.

Key responsibilities include determining patient appropriateness for home care, conducting thorough assessments, reviewing medication lists for accuracy, and providing direct care and treatments as ordered.

Documentation must be completed within agency timelines and quality standards, in collaboration with and under the guidance of a Clinical Field Supervisor.


Performance Responsibilities and Standards:

  • Ensure complete and accurate assessments and delivery of skilled PT services.

Determine patient appropriateness for home care.

Develop patient-centered plans of care based on comprehensive assessments.

Develop plans of care using an interdisciplinary approach.


Ensure that all documentation meets quality standards:
  • OASIS evaluations are conducted per CMS guidelines.
  • Documentation reflects PT care, treatments, and responses.
  • Documentation is clear and concise.
  • Documentation is completed in a timely manner.

Understand and follow PT code of ethics and standards of practice.

Coordinate with internal and external resources to meet patient needs.

Maintain established productivity expectations.

Communicate client care issues to the interdisciplinary team, including physicians.


  • Responsible for coordination of services.

Identify needs and refer to additional services as indicated to meet patient goals.

Report findings and plans to designated Case Managers/Care Teams and/or Clinical Supervisors.

Work in conjunction with Supervisors and designated staff to efficiently manage patients using a team approach.


  • Responsible for development of the care plan for paraprofessional staff relevant to patient care as applicable.

Supervise home health aides in performing designated tasks related to patient care and per care plan.

Communicate appropriate information to the home health aide department.


  • Comply with Nascentia Health policies and procedures and quality initiatives.

Maintain confidentiality.

Demonstrate knowledge and use of infection control practices.

Demonstrate knowledge of federal, state, and local government rules and regulations.

Demonstrate knowledge and compliance with agency policies and procedures.

Adhere to quality improvement standards.

Keep personnel requirements up-to-date, including health requirements, licensure requirements, and mandatory in-services.


  • Participate in personal growth and development.

Participate in educational in-services and meetings.

Maintain and update clinical knowledge on an ongoing basis.


  • Represent self and agency in a professional manner, including compliance with agency dress code.
  • Other duties as assigned.

Job Qualifications:
Currently registered by NY State and the American Physical Therapy Association and educational standard board as applicable
Current NY State Physical Therapy License.

Current and valid driver's license and auto insurance, and a vehicle which can be used for work (we do have a Fleet Program for field clinicians).

Self-directed and organized with the ability to work independently in a fast-paced environment.
Previous home care/case management experience a plus
Proficient computer skills (including knowledge of electronic medical records software) with ability to learn new software.
Exceptional oral, written, and verbal communication skills.
OASIS Certification, or willingness to obtain within 1 year of hire.
Minimum one year experience in Home Health Care required.

Physical Requirements:
Ability to sit or stand for long periods of time in a vehicle or in the field.

Ability to perform tasks involving physical activity (including but not limited to): Pushing/pulling on average 50 lbs with a max of 150 lbs; lifting/carrying on average 25 lbs with a max of 40 lbs; patient transfers, extensive bending, standing, kneeling, walking in various weather conditions and ability to go up and down stairs.

Ability to drive in

ALL
weather conditions (rain, sleet, snow, wind).
Visual/hearing ability sufficient to comprehend written or verbal communication
Ability to use telephone and communicate over the phone
Frequently uses a laptop computer to enter orders and obtain product information
Bloodborne Pathogens Exposure Category III - Employee who rarely has contact with blood and body fluids
