Candidate Profile

The ideal Community-Based Nurse Practitioner will possess the following qualifications:

  • Education:
    • A Nursing Degree or Nurse Practitioner Degree from an accredited institution in the U.S. is mandatory.
    • Completion of accredited training in nursing and nurse practitioner programs is essential.
    • An active, unrestricted medical license in Georgia is a prerequisite.
    • DEA Certification is required.
    • AANP Certification is necessary.
    • ANCC Certification is also required.
  • Experience:
    • A minimum of 3 years of experience as a Nurse Practitioner is expected, with a preference for experience in Health Home programs, particularly in managing chronic conditions.
    • Experience working with underserved populations facing socioeconomic challenges in accessing healthcare is highly valued.
    • Fluency in an additional language besides English is advantageous.
    • Skills in immunization and phlebotomy are necessary.
    • The Nurse Practitioner should be capable of performing specific in-office procedures safely within an outpatient context.
  • Entrepreneurial Spirit: CINQCARE aims to address longstanding gaps in healthcare delivery for Black and Brown populations. The Nurse Practitioner should demonstrate creativity and self-motivation to support this mission.
  • Communication Skills: Exceptional verbal and written communication abilities are required, with the capacity to present concepts clearly to diverse audiences, including non-native English speakers.
  • Relationship Management: Proficiency in building and maintaining effective relationships with patients, community leaders, and external stakeholders is essential.
  • Company Culture: A strong ethical foundation, sound judgment, and a collaborative spirit are crucial for success in this fast-paced and dynamic environment.

Job Responsibilities

The Community-Based Nurse Practitioner will report to the Chief Medical Officer and will be responsible for delivering clinical judgment, organization, and evidence-based analysis in alignment with CINQCARE's model of care. The candidate should embody CINQCARE's core values: Trust, Empathy, Commitment, Humility, Creativity, and Community Orientation.

The responsibilities include:

  • Providing comprehensive medical care to a diverse patient population, including adults, seniors, and children.
  • Conducting thorough evaluations, including medical history and physical examinations for risk assessments.
  • Addressing both chronic and acute healthcare issues while assessing medical urgency.
  • Ordering necessary diagnostic tests judiciously, considering resource limitations.
  • Developing and implementing individualized medical care plans.
  • Coordinating care and preventive services for targeted populations based on risk stratification.
  • Facilitating post-discharge coordination to minimize hospital readmissions and emergency room visits.
  • Conducting in-person visits across various settings.
  • Utilizing Telehealth platforms for synchronous video consultations as needed.
  • Prescribing medications when appropriate.
  • Maintaining accurate documentation in electronic medical records or other systems.
  • Collaborating with licensed physicians as required by state regulations.
  • Performing additional job-related duties as assigned.

Leadership and Collaboration

The Community-Based Nurse Practitioner will also be responsible for:

  • Leadership: Leading initiatives to define and implement strategies that create value for CINQCARE.
  • Strategic Improvement: Enhancing outcomes for CINQCARE's Family Members and improving practice efficacy.
  • Collaborative Solutions: Developing strategies to meet the clinical objectives of CINQCARE's model of care.
  • Clinical Expertise: Providing clinical knowledge and seeking specialty consultations as necessary, recognizing when physician oversight is required.
  • Work Environment: Fostering a collaborative, safe, and inclusive atmosphere for the team and the broader organization.

Work Environment and Physical Requirements

This role involves both in-home and office-based responsibilities, requiring frequent travel for home visits and visits to various healthcare facilities. The office work is conducted in a standard office environment with climate control and artificial lighting.

Frequent travel by car or public transportation is necessary, along with effective communication with patients, colleagues, and external partners. Proficiency in using communication devices is essential. The role may require extended periods of sitting and the ability to lift up to 50 lbs occasionally. Good manual dexterity for operating office equipment is also important.
