At APH Dependable Healthcare, we are seeking a highly skilled and compassionate Certified Nursing Assistant (CNA) to join our team as a Home Health Aide. As a CNA, you will play a vital role in providing personalized care and support to our clients in the comfort of their own homes.
Key Responsibilities- Provide safe and competent care to clients, following established plans of care and protocols.
- Assist clients with daily living activities, such as bathing, dressing, and grooming, to promote independence and dignity.
- Prepare and serve nutritious meals, as instructed by the nurse, to support clients' dietary needs.
- Assist clients with ambulation, as ordered by the physician and approved by the nurse, to prevent falls and promote mobility.
- Participate in rehabilitative processes, as directed by therapy personnel, to support clients' recovery and well-being.
- Encourage clients to engage in activities of interest, promoting mental alertness and stimulation.
- Provide emotional and psychological support to clients and their families, establishing trust and confidentiality.
- Report any changes in clients' mental or physical condition, or in the home environment, to the staff nurse or Aide supervisor.
- Perform routine housekeeping tasks to maintain a safe and comfortable environment for clients.
- Prepare visit reports and incorporate them into the clinical record on a weekly basis.
- Coordinate scheduling of visits with other staff members to ensure seamless care delivery.
- Collaborate with community agencies involved in clients' care, as directed by the nurse.
- Attend in-service training sessions, as required by regulation and agency policy, to stay up-to-date on best practices and industry developments.
- Notify the Agency of absences due to illness, emergency leave, or special professional meetings that may impact agreed service delivery.
- Certified Nursing Assistant (CNA) certification
- High school diploma or equivalent
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
- Ability to lift, bend, and move with ease
- Valid driver's license (if applicable)
